CORI Form - Required for ALL Coaches
Massachusetts Youth Soccer Association has been certified by the
Massachusetts Criminal System History Board, pursuant to M.G.L. c 6,
section 172H, for access to all existing criminal record information
( called CORI), including pending cases, convictions, and
non-conviction criminal case data. This information is not available
for dissemination for any purpose other than to further the
protection of children. This information will be maintained in
strict accordance with the directives of the Criminal System History
Board.
Massachusetts state law now mandates that we collect the following
information on every adult volunteer.
